For Prospective Patients
- Review the list of insurance that we accept.
- Please give us a call at +(775) 827-2400 and have your insurance information and a list of your current psychiatric medications, if applicable. Thank you for considering Memor Health.
Before Your First Appointment
- Please complete the electronic patient demographic information and consent letters by accessing the link sent to you by a member of our intake team.
- If you are unable to access and complete the electronic forms, please bring the following items to your first appointment: photo ID, insurance card, and a list of current medications, including dosage, schedule, and frequency, if applicable.
- Please arrive 15 minutes before your scheduled appointment.
Cancellation policy: Please be mindful that the time for your sessions is reserved specifically for you. If you must cancel a scheduled appointment, PLEASE CONTACT US AT LEAST 48 HOURS BEFORE THE APPOINTMENT to avoid being charged the late fee of $100.00. This fee must be paid before we can reschedule an appointment for you.
